Reception Forms Guide

Overview

Reception Forms allow you to create and manage intake forms, questionnaires, and patient registration forms that can be filled out at reception or by patients.

Accessing Reception Forms

URL: /clinic/reception-forms
Navigation: Sidebar → Reception Forms

Creating a Form

  1. Click Create Form
  2. Enter form name and description
  3. Add form fields:
    • Text fields
    • Dropdown menus
    • Checkboxes
    • Radio buttons
    • Date pickers
  4. Set field requirements
  5. Save form

Using Forms

  • Assign forms to patients
  • Forms can be filled online or in-clinic
  • View completed forms in patient profile
  • Export form data

Best Practices

  • Create forms for common intake needs
  • Keep forms concise and clear
  • Review completed forms before appointments
