Reception Forms allow you to create and manage intake forms, questionnaires, and patient registration forms that can be filled out at reception or by patients.

Creating a Form
- Click Create Form
- Enter form name and description
- Add form fields:
- Text fields
- Dropdown menus
- Checkboxes
- Radio buttons
- Date pickers
- Set field requirements
- Save form
Using Forms
- Assign forms to patients
- Forms can be filled online or in-clinic
- View completed forms in patient profile
