Required documents checklist
- Articles of organization / professional corporation papers for that state
- EIN from IRS; state tax accounts
- State PT licences for every treating therapist (compact licence if you use PT Compact)
- NPI (individual and organizational) if you will bill insurers
- Malpractice and general-liability insurance
- Local business licence, occupancy, and fire inspection
- HIPAA policies, BAA with vendors, and a designated privacy officer
- Medicare/Medicaid enrollment only if you will bill those programmes (additional survey in some cases)
- CLIA only if you perform laboratory tests on site
Facility & equipment requirements
- Licensing is state-by-state. California, New York and Texas have extra corporate-practice and clinic rules.
Staff licensing
- PTAs must be supervised per that state’s practice act; using the title Physical Therapist without a state licence is illegal.

Important tip
Confirm corporate-practice-of-medicine / PT-ownership rules before a non-PT investor joins. That is a frequent reason banks and payers freeze enrollment.
