Required documents checklist
  • Application to the Health Directorate (treatment-institutions / private facilities unit)
  • Commercial registry extract (السجل التجاري) — individual firm or LLC
  • Tax card (البطاقة الضريبية) and VAT file if the turnover threshold applies
  • Lease registered at the real-estate office, or title deed
  • National ID copies of owner and technical director + personal photos
  • Criminal-record certificates (فيش وتشبيه) for owner and technical director
  • Syndicate membership card + MOH practice licence of the technical director
  • Syndicate no-objection / appointment letter naming the technical director
  • Staff roster with each PT’s syndicate card and practice licence
  • Architectural floor plans (مساقط أفقية) matching the actual fit-out
  • Civil Defence / fire-safety approval for the premises
  • Environmental-health / sanitation inspection report
  • Equipment inventory (electrotherapy, exercise, assessment) matching the licensed activity
  • Social-insurance registration for employed staff (التأمينات الاجتماعية)
  • Chamber of Commerce certificate when the legal form is a company
Facility & equipment requirements
  • Separate treatment cubicles or rooms with privacy (curtains/doors) and adequate lighting/ventilation
  • Accessible WC; waiting area not used as a treatment space
  • Hand-wash points and consumables store; biomedical waste pathway if you use needles/electrodes
  • Clinic signboard must match the licensed name and must not imply a medical specialty you are not licensed for
  • Do not advertise “hospital” or surgical services; activity is physiotherapy / rehabilitation only
Staff licensing
  • Every treating physiotherapist: syndicate membership + MOH practice licence (ترخيص مزاولة)
  • Technicians / aides cannot independently diagnose or prescribe a physiotherapy plan
  • Foreign PTs need degree equivalency + syndicate pathway before they can be listed on the licence
Licensing steps — in order

Choose legal form: solo clinic under your name, or a company/centre with a technical director

Issue commercial registry + tax card, then sign a premises contract that allows a health activity

Prepare stamped drawings and start Civil Defence + environmental-health files in parallel

Appoint the technical director and collect syndicate papers for every PT on staff

Submit the Health Directorate file; attend inspection; close any fit-out remarks

Collect the operating licence, display it, and keep equipment/staff files ready for re-inspection

Important tip

Start Civil Defence and drawings before you buy equipment. Most delays are a mismatch between the drawing and the actual rooms, or a technical director who is already named on another licence.
